<p>Stepping on the scale each day and diligently recording their caloric intake and body weight in a little booklet, my patients marked the progress and failures of their dieting efforts. It was the mid-1990s and I was working as a medical fellow in endocrinology and metabolism at the University of Düsseldorf Medical Center in Germany. During the day, we optimized insulin therapy in patients with Type 1 or Type 2 diabetes, using modern tools such as new insulin analogs, glucose meters, and insulin pumps. In the evening, I supervised an outpatient obesity clinic. Our department was internationally renowned as an accredited World Health Organization collaborating center, and patients came from far and wide to seek care for their diabetes, and a variety of obesity-related conditions.</p>

Some hormones, such as insulin, thyroid hormone, or cortisol, are “major players,” and their deficiency or excess can result in life-threatening metabolic derangements. Others, such as calcitonin, pancreatic polypeptide, or amylin can be viewed as complementary signals that enhance, or “fine-tune,” a tightly regulated metabolic process. In many cases, the central nervous system (CNS) orchestrates and balances these hormonal interactions, serving as the role of conductor&#8230;.</p>

Other aspects of menopause carry real health risks. 

A change in hormone levels means the increased likelihood for a number of serious health conditions such as osteoporosis and diabetes. So even if a woman is lucky enough not to experience hot flashes, she still needs to talk to her doctor. 

"A lot of women make the mistake that because they're symptom free, or their symptoms have gone away, they're not at risk for [other health issues], " said Dr. Jeff Romig, a Northeast Ohio endocrinologist who specializes in bioidentical hormone therapy. (Such therapy uses hormones that are biologically identical to hormones naturally produced in the body.)

<div style="MARGIN-TOP: 6px">The Change, the Big M, the Private Summer.Although the euphemisms aren&#8217;t spoken in hushed tones nearly as much as they once were, menopause &#8212; the permanent cessation of reproductive fertility &#8212; means different things to different women: freedom from birth control, the loss of conceiving naturally, a new chapter of life.</p>
<p>The median age for menopause, which officially begins 12 months after the last menstrual cycle, is 51. Which means the country&#8217;s 39.3 million female baby boomers are either fast approaching or in the throes of menopause. It&#8217;s no coincidence, then, that more research money is being targeted for treatments and studies on how menopause affects women.</p>

<p><strong>The ability to mount an immune response to influenza A (H1N1) infection is significantly compromised by a low level of arsenic exposure that commonly occurs through drinking contaminated well water, scientists at the Marine Biological Laboratory (MBL) and Dartmouth Medical School have found.</strong></p>
<p>Joshua Hamilton, the MBL&#8217;s Chief Academic and Scientific Officer and a senior scientist in the MBL&#8217;s Bay Paul Center; graduate student Courtney Kozul of Dartmouth Medical School, where the work was conducted; and their colleagues report their findings this week in the journal <em>Environmental Health Perspectives</em>.</p>
<p>&#8220;When a normal person or mouse is infected with the flu, they immediately develop an immune response,&#8221; says Hamilton, in which <a rel="tag" href="http://www.physorg.com/tags/immune+cells/" target="_blank">immune cells</a> rush to the lungs and produce chemicals that help fight the infection. However, in mice that had ingested 100 ppb (parts per billion) arsenic in their <a rel="tag" href="http://www.physorg.com/tags/drinking+water/" target="_blank">drinking water</a> for five weeks, the immune response to H1N1 infection was initially feeble, and when a response finally did kick in days later, it was &#8220;too robust and too late,&#8221; Hamilton says. &#8220;There was a massive infiltration of immune cells to the lungs and a massive <a rel="tag" href="http://www.physorg.com/tags/inflammatory+response/" target="_blank">inflammatory response</a>, which led to bleeding and damage in the lung.&#8221; Morbidity over the course of the infection was significantly higher for the arsenic-exposed animals than the normal animals.</p>
<p>Respiratory infections with <a rel="tag" href="http://www.physorg.com/tags/influenza/" target="_blank">influenza</a> A virus are a worldwide health concern and are responsible for 36,000 deaths annually. The recent outbreak of the influenza A H1N1 substrain (&#8220;<a rel="tag" href="http://www.physorg.com/tags/swine+flu/" target="_blank">swine flu</a>&#8220;), which is the same virus that Hamilton and his colleagues used in their arsenic study, to date has killed 72 people in Mexico and 6 in the United States.</p>
<p>&#8220;One thing that did strike us, when we heard about the recent H1N1 outbreak, is Mexico has large areas of very high arsenic in their well water, including the areas where the flu first cropped up. We don&#8217;t know that the Mexicans who got the flu were drinking high levels of arsenic, but it&#8217;s an intriguing notion that this may have contributed,&#8221; Hamilton says.</p>
<p>The U.S. Environmental Protection Agency considers 10 ppb arsenic in drinking water &#8220;safe,&#8221; yet concentrations of 100 ppb and higher are commonly found in well water in regions where arsenic is geologically abundant, including upper New England (Massachusetts, New Hampshire, Maine), Florida, and large parts of the Upper Midwest, the Southwest, and the Rocky Mountains, Hamilton says.</p>
<p>Arsenic does not accumulate in the body over a lifetime, as do other toxic metals such as lead, cadmium, and mercury. &#8220;Arsenic goes right through us like table salt,&#8221; Hamilton says. &#8220;We believe for arsenic to have health consequences, it requires exposure day after day, year after year, such as through drinking water.&#8221;</p>
<p>Arsenic exposure not only disrupts the innate immune system, as the present study shows, it also disrupts the endocrine (hormonal) system in an unusually broad way, which Hamilton&#8217;s laboratory discovered and first reported in 1998.</p>
<p>&#8220;Most chemicals that disrupt hormone pathways target just one, such as the estrogen pathway,&#8221; he says. &#8220;But arsenic disrupts the pathways of all five steroid hormone receptors (estrogen, testosterone, progesterone, glucocorticoids, and mineralocorticoids), as well as several other hormone pathways. You can imagine that just this one effect could play a role in cancer, diabetes, heart disease, reproductive and developmental disorders-all the diseases that have a strong hormonal component.&#8221;</p>
<p>At this point, Hamilton thinks arsenic disrupts the innate immune system and the endocrine system through different mechanisms. &#8220;Arsenic may ultimately be doing a similar thing inside the cell to make these effects happen, but the targets are likely different,&#8221; he says. The proteins that mediate hormone response are different than the proteins that mediate the immune response. &#8220;We don&#8217;t yet know how arsenic disrupts either system at the molecular level. But once we know how it affects one system, we will have a pretty good idea of how it affects the other systems as well.&#8221;</p>

After graduating from Michigan State University in 1990, she traveled, earned a law degree and began working as an attorney. When Harper married in 2006, she and her husband, Jeff, hoped a baby would soon follow. </strong></p>
<p>&#8220;We didn&#8217;t marry until I was 38,&#8221; Harper says, &#8220;and we always knew we didn&#8217;t have a lot of time to waste.&#8221;</p>
<p>A year passed; no baby.</p>
<p>Like many women who marry later in life, Harper didn&#8217;t think much about her fertility until she&#8217;d reached the age at which many doctors warn that healthy pregnancies don&#8217;t come easily.</p>
<p>&#8220;What a lot of women do not know is that we are born with all of the <span style="color: #000000;">eggs</span> that we are going to get. And a lot of those eggs are used before we get out of <a rel="tag" href="http://www.physorg.com/tags/puberty/" target="_blank"><span style="color: #0e3266;">puberty</span></a>,&#8221; says Kim Hahn, founder and CEO of Conceive magazine. &#8220;We are not like men; we do not regenerate eggs.&#8221;</p>
<p>Concern about her age prompted Dawn Crowley&#8217;s doctors to encourage her to get pregnant even before her planned marriage.</p>
<p>&#8220;When I told my doctor we wanted to have children after we married, she looked at me and said, &#8216;I understand the need to be married, but you&#8217;re 38 years old. I suggest you try right now,&#8217;&#8221; recalls Crowley, who is now 40.</p>
<p>The Birmingham, Mich., resident married Jim Crowley, 50, in August 2007. They gave birth to a healthy 7-pound girl, Kate, six months later.</p>
<p>The statistics might surprise some women:</p>
<p>At 20, the odds of getting pregnant when everything is timed perfectly are about 25 percent per cycle. By age 30, the odds drop to 10 percent to 15 percent each month, and by age 40, it&#8217;s 5 percent, reports Conceive magazine.</p>
<p>Women who get pregnant between the ages of 35 and 45 face a 20 percent to 35 percent chance of miscarriage, according to the American <span style="color: #000000;">Pregnancy </span>Association. That&#8217;s on top of common complications such as <a rel="tag" href="http://www.physorg.com/tags/high+blood+pressure/" target="_blank"><span style="color: #0e3266;">high blood pressure</span></a> and gestational <a rel="tag" href="http://www.physorg.com/tags/diabetes/" target="_blank"><span style="color: #0e3266;">diabetes</span></a>.</p>
<p>By the time a woman is 43, the risk of having a baby with <a rel="tag" href="http://www.physorg.com/tags/chromosomal+abnormalities/" target="_blank"><span style="color: #0e3266;">chromosomal abnormalities</span></a> such as Down&#8217;s syndrome rises to 1 in 50, from 1 in 1,500 at age 20, according to pregnancyinfo.net</p>
<p><!-- inj G3 -->At age 35, the rate is 1 in 350.</p>
<p>The rate of in-vitro fertilization has increased 17 percent from 2003 to 2007, with the greatest increase seen among women ages 35 to 37, according to the Society for Assisted Reproductive Technology. But age also seems to play a role in the success of the procedure. Younger women &#8212; those younger than 35 &#8212; have about a 40 percent chance of having a live birth with IVF. By the time a woman is 42, the live birth rate with IVF drops to about 11%.</p>
<p>Children born to older parents are more likely to develop autism. That&#8217;s especially true for first-born kids, according to a study in the December issue of the American Journal of Epidemiology. Also, a Swedish study in the American Journal of Psychiatry found that children with fathers older than 45 were twice as likely to develop schizophrenia.</p>
<p>The trend of women having babies later in life shows no sign of slowing down. The National Center for Health Statistics reports that the percentage of women giving birth for the first time at age 35 or older has increased eight-fold since 1970 &#8212; from 1 percent to 8 percent.</p>
<p>The birth rate among women ages 30 to 34 grew 2 percent from 2006 to 2007. Among women 40 to 44, the birth rate grew 1 percent to 9.5 births per 1,000 women &#8212; one of the highest rates ever.</p>
<p>&#8220;It makes sense that women are waiting longer to start families &#8212; college, careers, not meeting the right person earlier in life,&#8221; says Dr. Kristen Wuckert, an ob-gyn at Mission Obstetrics and Gynecology in Warren, Mich. Years ago, she says she might have seen an older woman once a week; now it&#8217;s a daily or twice-a-day occurrence.</p>

The segment entitled &#8220;Contamination&#8221; left the viewer with the impression that every child who gets measles will die and that MMR vaccine poses no risks whatsoever to any child. It was a cheap shot at parents with vaccine injured children trying to protect their healthy children from vaccine injury and yet another wake-up call for Americans, who understand the importance of voluntary, informed consent to taking any pharmaceutical product or undergoing any medical procedure that can injure and kill.As 2008 ended and the New Year began, evidence that the &#8220;Tipping Point&#8221; in the 27-year old vaccine safety and informed consent movement has arrived is becoming more apparent. There is the escalating rhetoric and hyperbole being spewed by the <a href="http://rs6.net/tn.jsp?e=001_-MDWMAk9LQCvb9wJeZtwGTYBoaCu9d2uxkRGamfowC4QIIHG3FYC86chWvsUYoN83YCd_JCamNK41-GOe8tXnegBg_RouKuTDKUUMPglmilLF0odz-kBQN9_XtgPdj1eEd-wrsZvY6FW0et2ScMaZUJML5T6RZkQLiCM0dXrRbhgJC0rNFh_w==" target="_blank">evangelistic pediatrician </a>from Children&#8217;s Hospital of Philadelphia, who called for doctors to be burned at the stake for the heresy of individualizing CDC and AAP vaccine use guidelines. In a <a href="http://rs6.net/tn.jsp?e=001_-MDWMAk9LRN4BlvSPouHT-X0pqV6cGC1q6pqnX4TZWPbZVaT879KGuVhs5H1nxvCy-QP97gI6eUr0ggJYaVKvdUD6oHiZuw1AEk21WeB9fWwTNII66vXTRCkQd_z_5-VvTK-AC0rYU4dH1ZBzNimhgHTgRhbi75p3V95zKT1EK_0dPrccSZBVEvdG7U40OY" target="_blank">brilliant letter </a>to Pediatrics, it was a parent of a vaccine injured child, neurologist Jon Poling, M.D., who pointed out why sophomoric errors in logic and misunderstanding of the scientific method invalidated the claims of Paul Offit. Soon Dr. Poling was joined by others speaking out in defense of pediatricians like <a href="http://rs6.net/tn.jsp?e=001_-MDWMAk9LSfXn014I8gey7sTTidw74fPiknyrfFNRqUbYIDLMY4ZMWbFoM5B9PWXE645CGhF4rbWtQwlS7c80G0hOcnZ9kRvnDsZ3bPUdbiL1hropPVmQUdj4lhOyvKsr6g3Yvgmwc=" target="_blank">Bob Sears, M.D.</a>, who are wisely advocating that doctors and parents engage in shared vaccine decision-making for children.</p>
<p>Then there was the stern reminder to parents from militant New Jersey health officials that babies <a href="http://rs6.net/tn.jsp?e=001_-MDWMAk9LScFdNDVTjzSNWy76e3Xg9cLFSBZ4fWp0Nl64iV6PN44CClPIkXpbzsLcjmhedrMWhZIZqHIG32mJ4qcEVyUWWO-WrSU8OEm3Y1pRnZaqMG_fBSL7D8l-BAEEPZU_zu-B3sA1mXDO5hdSJo_lH0e-3TDF7gDIRJOAF0p5vbdPMHxBXI8gJvI5K131J5HDeY4cvJv3lC1VRC4YixvdiJo_6u" target="_blank">must get flu shots</a> by January 2009 or face being kicked out of daycare and pre-school. Parents living in the state with the most vaccine mandates (41 doses of 13 vaccines), especially those with <a href="http://holyhormones.com" target="_blank">children already suffering with health problems</a>, are trying to protect their children from further damage but have no recourse because New Jersey state law only allows restrictive medical and religious exemptions. Vaccine educated parents are writing letters to legislators urging them to pass a <a href="http://rs6.net/tn.jsp?e=001_-MDWMAk9LTrP3XpdXc64iTJpSm4geK8Wvt38zrXZZufM6uAx9Qc1_DUHM9lICeGipD9SJVt4LvzO3EUsHCUjFNca56sdtJ2kw18CFrRM-h2mXp80oanK0y5ZBBOujs6" target="_blank">conscientious belief exemption </a>to vaccination.</p>
<p>After a Dec. 14 legislative roundtable discussion at Stony Brook University with proponents for and against addition of a philosophical exemption to New York vaccine laws, vaccine educated parents are urging that letters be written to legislators (New York Assembly at <a href="http://assembly.stat e.ny.us/ " target="_blank">http://assembly.stat e.ny.us/ </a>and New York Senate at <a href="http://www.senate.state.ny.us/senatehom epage.nsf/senators" target="_blank">http://www.senate.state.ny.us/senatehom epage.nsf/senators</a>OpenForm and New York Senate at <a href="http://assembly.stat e.ny.us/" target="_blank">http://assembly.stat e.ny.us/</a> and New York Senate at <a href="http://www.senate.state.ny.us/senatehom epage.nsf/senators" target="_blank">http://www.senate.state.ny.us/senatehom epage.nsf/senators</a>OpenForm ) and also to New York Governor David Paterson, Executive Chamber, State Capitol, Albany, NY 12224 in support of the exemption.</p>
<p>Merck rang in the New Year trying to pump up sales of the reactive Gardasil vaccine by asking the FDA to open up the <a href="http://rs6.net/tn.jsp?e=001_-MDWMAk9LT1y4K5edlzWhqk0pfPxOEvfbxRL3Yo_QFQ02KqBntk_ojNm2TxVe5lPXw46CD2FUFcvgpZSH-B48HvoKg2rJjinjVg8wCmOn2YRb4qVgMQZlSgpGQDnMT-Kj95zTBNmRo=" target="_blank">young boy market</a>, while Prevnar-maker Wyeth is making a bid to become the world&#8217;s <a href="http://rs6.net/tn.jsp?e=001_-MDWMAk9LT6SJAj9OW_MMRwxFn10eHdSkFCmlCo1TxfKf_8CtFeSWfChRCjYh_rrTJi74a9UNM4s5rT3q198tqv7pTEl0Lm-9tha9QhvMpgkasMl7tOi0DBai_zIuq0kLHvJ7yiG8tyHNRKrWkXapLtN2UD6yFW0xXUDpkxi9obaNkStCFIWZ6iodlziBIZvTEoSFmES_eDwZaeLpbfVg==" target="_blank">most profitable vaccine manufacturer</a>. The Jan. 1 voluntary moratorium by drug companies on giving doctors free pens and trinkets is a token drop in the bucket compared to the <a href="http://holyhormones.com" target="_blank">big bucks</a> Big Pharma will spend in 2009 to keep doctors convincing Americans that the only way they can stay healthy is to consume more and more drugs and vaccines.</p>
<p>But then came a breath of fresh air this week when the formidable M.I.N.D. Institute at UC-Davis published an <a href="http://rs6.net/tn.jsp?e=001_-MDWMAk9LQLoxnmcSBPghF9ICRHa95Ii1A9On4kNRVmjVjbRrhfoasZ_09BgurOvtJ3-QvIAlE-hJaGAA-_OUjD84K5KYz4Hq1iEz_3Inm8q2_2VmwzCDIcaUkRdf4RHGKlI2o8zTDZiUBHiEx-QS5mU1EWKDIPmUsyBsqYqKU=" target="_blank">epidemiological study</a> confirming that the autism epidemic that has emerged during the past three decades is not due to genetics or &#8220;better counting&#8221; but is likely caused by environmental co-factors.</p>
<p>2009 is off to a contentious, challenging and promising start. Like all social movements that advocate reform of institutions stubbornly resistant to change, the way is not easy and change does not come quickly or without sacrifice. The three-decade U.S. vaccine safety and informed consent movement is approaching its final and most important stage: the fight for freedom of choice. As several hundred <a href="http://rs6.net/tn.jsp?e=001_-MDWMAk9LRYNu4yzPAqawlkB86xSFFaYw-MLhAzU7xqllrMYnaS1B2tAS2idUZ1Ov0we3S6tGHIpJJ31ctzQpSy01Xh5IpFRxJFZFvWf2yRaYc0Tzo8RwikE8joPjWT" target="_blank">vaccines are being developed and tested </a>in several thousand clinical trials around the world and drug companies are jockeying for market-share positions, doctors are trying harder to convince Americans that 69 doses of 16 vaccines given to children between birth and age 18 will keep America healthy.</p>
<p>The argument that more vaccination will equal better health is an argument that is getting harder to make as <a href="http://rs6.net/tn.jsp?e=001_-MDWMAk9LQz1SyL0eZuCj5O0eHZkUBJaxucuB-wwhFJVDJ5VK4-awqhs_5x_itYiohb92P58Bs8shdoGUgAPqFnWqndGpxgnTnVqwiOpaqFcBkPum6njnjLXU_Mprj-f51PVNqxOosPlOTz5Uo8pzUEzUoSRvkc" target="_blank">one in two Americans </a>suffers from chronic disease and America plummets to <a href="http://rs6.net/tn.jsp?e=001_-MDWMAk9LQOjNYCk3UkG5vRJeY6_xD2I7pNj4wLsdzNu5cigRzfwTg89t-mIpPzCok31LosGVBQpLNsLLC5hXLSUAeLwRO6T9ZWVuwbmDahZ2khGSumC8IMPsZPywSI1ypkD1osWKJo6qZu9vbmiZMY1FCk53_A1EKOEk2801Y=" target="_blank">39th in infant mortality </a>while 25 percent of all children are suffering with learning disabilities, ADHD, severe allergies, autism, asthma, diabetes, inflammatory bowel disorder, rheumatoid arthritis and other chronic immune and brain disorders. The question that doctors and public health officials fear most is:</p>
<p>WHY IS THE MOST HIGHLY VACCINATED CHILD POPULATION IN THE WORLD SO SICK AND DISABLED?</p>
<p>Until that question is answered by doctors and public health officials, there aren&#8217;t enough daytime or evening soap operas, bogus news reports and poorly conducted studies claiming that vaccines are totally safe, or threats by vaccine patent holders and doctors shouting &#8220;coincidence&#8221; every time another child regresses into chronic poor health after vaccination to keep the tipping point from tipping.</p>

<p>&#8220;Sears&#8217; book, published in October 2007 as part of the Sears Parenting Library, has already sold more than40,000 copies and has moved into the top 100 on the Amazon.com bestseller list. The popularity of Sears&#8217; book centers in part on 2 schedules, called alternative and selective, that offer parents a way to avoid giving their children several vaccines at one time. Sears&#8217; book is unique. Unlike typical antivaccine books, he offers a middle ground, allowing parents to act on their fears without completely abandoning vaccines. Unfortunately, Sears sounds many antivaccine messages&#8230;.At the heart of the problem with Sears&#8217; schedules is the fact that, at the very least, they will increase the time during which children are susceptible to vaccine-preventable diseases. If more parents insist on Sears&#8217; vaccine schedules, then fewer children will be protected, with the inevitable consequence of continued or worsening outbreaks of vaccine-preventable diseases. <p>&#8220;In their assault on Dr. Sears, Offit and Moser confuse scientific methodology. Even more dangerous than not having an understanding of science, is the presumption that one does &#8220;grasp the scientific method.&#8221; Actually, in designing an epidemiological study, one must have a good estimate of the effect size, in order to determine the power of a study. Offit misuses the statistical term &#8216;power&#8217; to suggest that this allows one to &#8220;reject or not to reject the null hypothesis.&#8221; This is incorrect&#8230;..As physicians we took an oath to &#8216;first do no harm&#8217; to our individual patients. Dr. Sears offers a pro-vaccine individualized approach to childhood immunization that acknowledges the risks, benefits, and uncertainties of this medical intervention. Dr. Sears should be applauded for his efforts to provide safe vaccination alternatives to his patients, given the void of randomized controlled trials to support continued growth of the current CDC/AAP schedule. Rather than personal attacks, let&#8217;s turn to science to provide the answers. The enormous public benefit of vaccination cannot be used to stifle open discourse on critical vaccine safety issues. <p>&#8220;The state&#8217;s new law requiring young children attending licensed pre-school and child care centers to get flu vaccinations will be tested this week when thousands of children return to classrooms and playrooms after the long holiday break. New Jersey, the first state in the nation to require flu shots for young schoolchildren, set a Dec. 31 deadline for parents to obtain flu vaccinations for their children. It was part of a new policy requiring a total of four additional immunizations for schoolchildren over the objections of some parents who worry about possible risks from vaccinations.
